Injections for Rotator Cuff Shoulder Pain – What does the evidence say?

Rotator cuff related shoulder injuries are one of the most common causes of shoulder pain and dysfunction, particularly in individuals over the age of 50.

These injuries not only lead to pain and limited mobility but also interfere with daily tasks such as dressing, lifting, or reaching overhead. With increasing numbers of patients opting for non-surgical treatment, drug injection therapies into the shoulder joint have become widely used. But injections are only part of the solution—physiotherapy is essential to restore movement, strength, and long-term shoulder health. Here at FightingFit, we want to make sure we optimize both options to have the best outcomes for each patient.

Recent research, including a network meta-analysis in 2022 by Zhi et al. reviewed 10 high-quality RCTs with 861 patients, has compared four main types of injection therapy for rotator cuff injuries:

  • Corticosteroid injections (COR)
  • Platelet-rich plasma (PRP)
  • Hyaluronic acid (HA)
  • Prolotherapy (PRO)

     

This blog explains each type of injection, highlights the latest research, and outlines how physiotherapy complements these treatments to achieve optimal outcomes.

 

1. Corticosteroid Injections (COR)

Corticosteroids: Anti-inflammatory and pain relief-focused

Corticosteroid injections are widely used for rotator cuff-related conditions, especially in the early stages. They are effective in reducing inflammation in the subacromial space and surrounding soft tissues, providing relatively quick pain relief.

Research Highlights:

  • Effective in the short term for reducing pain and improving mobility.
  • The meta-analysis showed steroid injections combined with hyaluronic acid (COR + HA) were the most effective among all therapies studied, with an SUCRA score of 84.8%, indicating top-ranked effectiveness.
  • Clinical guidelines (e.g. AAOS and Chinese Orthopaedic guidelines) support their use, although they recommend cautious dosing due to the risk of tendon weakening with repeated use.

How Physiotherapy Supports COR Treatment:

  • Early-stage mobility exercises: Once pain is reduced, therapists reintroduce gentle movement to prevent stiffness.
  • Scapular and rotator cuff strengthening: Physiotherapists use graduated resistance exercises to prevent recurrence and build endurance.
  • Postural correction and activity education: Addressing mechanical causes (e.g. poor ergonomics or movement patterns) is key for long-term success.

Clinical Tip: The best results are seen when corticosteroid injections are followed by a structured 6–8 week physiotherapy program.

2. Platelet-Rich Plasma Injections (PRP)

PRP: Biologic healing and tendon regeneration

PRP involves drawing the patient’s own blood, concentrating platelets, and injecting the solution into the injury site. This delivers growth factors that promote tendon healing, vascularisation, and cell regeneration—making PRP particularly valuable for degenerative or chronic rotator cuff tendinopathy.

 

Research Highlights:

  • PRP was ranked mid-range in effectiveness in the network meta-analysis, performing better than PRO or HA alone, but not as high as combination therapies.

     

  • Especially beneficial in the long-term compared to corticosteroids, which have faster but often shorter-lasting effects.

     

How Physiotherapy Supports PRP Treatment:

  • Load management: PRP benefits from relative rest initially, followed by progressive loading.

     

  • Eccentric and isometric tendon loading: Physiotherapists design exercise programs to stimulate tendon repair without overloading.

     

  • Individualised rehab timelines: As PRP’s effects are gradual, therapists adjust pacing based on tissue response and healing stages.

     

Clinical Tip: PRP is ideal for active individuals or athletes looking for a non-surgical healing approach, but physio is essential to stimulate and guide tendon adaptation post-injection.

 

3. Hyaluronic Acid Injections (HA)

HA: Lubrication and joint protection

Hyaluronic acid acts as a joint lubricant, mimicking synovial fluid and reducing friction within the joint. While traditionally used for osteoarthritis, HA has shown benefit in reducing shoulder pain and improving comfort during movement in rotator cuff-related issues.

Research Highlights:

  • HA alone was the least effective in the network analysis for rotator cuff injury when compared to other therapies.

     

  • However, when combined with corticosteroids (COR + HA), it became the most effective therapy overall.

     

  • HA’s main advantage is its protective effect on cartilage and its lower side-effect profile compared to corticosteroids.

     

How Physiotherapy Supports HA Treatment:

  • Mobility and functional integration: With reduced joint friction, patients can tolerate a wider range of movement, allowing therapists to focus on shoulder mechanics.

     

  • Neuromuscular re-education: With less joint irritation, proprioception and coordination exercises can be introduced earlier.

     

  • Post-injection functional progression: Therapists focus on helping patients reintegrate movement into real-world tasks without discomfort.

     

Clinical Tip: While HA injections alone may have limited benefit, they play a strong supporting role when paired with physiotherapy and other therapies like COR.

 

4. Prolotherapy (PRO)

PRO: Stimulating the body’s healing response

Prolotherapy involves injecting a concentrated dextrose solution into ligaments or tendon insertions, creating a small controlled inflammatory response to stimulate tissue healing. This approach is still emerging but shows promise in chronic rotator cuff injuries or partial tears.

Research Highlights:

  • PRO ranked lower in efficacy compared to PRP and COR but showed better results than HA alone.

     

  • Some studies report significant improvement in pain and function over 6–12 months post-treatment.

     

  • Particularly useful when other conservative treatments fail or are contraindicated.

     

How Physiotherapy Supports PRO Treatment:

  • Inflammation management: In early stages, physiotherapists monitor for overuse and modify activity levels.

     

  • Gradual tissue loading: As collagen remodelling occurs, strength and mobility are progressively restored through exercise.

     

  • Function-focused rehab: Therapists focus on returning patients to work, sport, or daily function with improved resilience.

     

Clinical Tip: Prolotherapy outcomes are greatly enhanced by a carefully managed, longer-term physiotherapy program.

 

Conclusion: Injections Are Not the End – They’re the Beginning

The evidence is clear: shoulder injections can significantly reduce pain and improve mobility, but only when combined with targeted physiotherapy do patients achieve lasting recovery.

The most effective therapy according to the meta-analysis?
The analysis showed that Corticosteroid + Hyaluronic Acid injection was the most effective, followed closely by PRP + Corticosteroid.

But regardless of the injection type, physiotherapists play a central role by:

  • Restoring range of motion

     

  • Strengthening the rotator cuff and scapular muscles

     

  • Addressing postural and biomechanical faults

     

  • Tailoring rehab based on the specific injection used and patient needs

     

As more evidence emerges, the integration of evidence-based injection therapy with structured physiotherapy programs remains the gold standard for conservative rotator cuff injury management.

Be AWARE they’ll immediately try to commit you to multiple follow up sessions after your initial consultation. This happened to me and then I later found out that as it just so happens, they have a cancellation policy that says you have to pay 100% of the consultation fee for cancelling. They failed to tell me this when they were pushy in committing me to 6 sessions after my consult, so that was a really nice little surprise.

I was already uncomfortable with how pushy they were in having me commit to so many sessions, so when I heard about the policy I just cancelled all together. So, I can’t speak further to the quality of their service throughout therapy phase. FYI this was also a few months ago now, it’s possible (though unlikely) they may have changed the policy. Just ask the question.
